RNs and LVNs are available 24 hours per day, 7 days per week to provide the following services on an intermittent (per visit) basis:
- Patient care services
- Skilled assessment and monitoring
- Care management and coordination with other disciplines/resources
- Skilled instruction to patients/families (e.g., medication, new diagnosis onset, nutrition/special diets)
- Chronic disease management (e.g., diabetes, CHF, COPD, urinary incontinence)
- Wound care
- Ostomy care
- Tracheostomy care
- Pain management/symptom control
- Infusion services (peripheral and central lines)
- Nutritional support (TPN, PPN, enteral feedings)
- Medication administration (injection/IV)
- Laboratory specimen procurement
- Care to the terminally ill
- Care to the patient receiving oxygen/ventilator assistance
- Post inpatient and outpatient surgery services.
