Choosing a name for your baby is very special. After your child is born, an Admissions Representative will contact you in your hospital room. This interview enables us to review and verify the information you have provided, check for any errors or changes, and complete any missing information.
As a courtesy to our new parents, we can help you get a social security number newborn. In order to do so, we must have your baby's name at the time the birth certificate is completed. Your child's social security card will be forwarded to you within five to eight weeks by the Social Security Administration Office. To ensure that the birth records process goes smoothly, we would like you to fill out the Birth Certificate Registration form and send it with your Preadmission Questionnaire. This will ensure that the birth records office has all of the information needed to prepare the birth certificate for your child.
