RNs and LVNs are available 24 hours-per-day, 7 days-per-week to provide the following services on an intermittent (per visit) basis.

Patient Care Services

Skilled Assessment & Monitoring
Care management & coordination with other disciplines/resources
Skilled instruction to patients/families (e.g. medication, new diagnosis onset, nutrition/special diets)
Chronic Disease Management (e.g. diabetes, CHF, COPD, urinary incontinence)
Wound Care
Ostomy Care
Tracheostomy Care
Pain Management/Symptom Control
Infusion Services (peripheral and central lines)
Nutritional Support (TPN, PPN, Enteral feedings)
Medication Administration (injection/IV)
Laboratory Specimen Procurement
Care to the Terminally Ill
Care to the patient receiving oxygen/ventilator assistance
Post inpatient and outpatient surgery services.
