We strive to provide optimal care during your stay. However, if you have concerns about your care, you may contact one of our Service Representatives. They may be called at (714) 771-8000, extension 11000, 8 a.m. to 4 p.m., Monday through Friday. Messages may be left at this number at any time. Return calls or visits from the Service Representatives will be made by the next working day (Monday through Friday). If you have an immediate need, please don’t hesitate to call the hospital operator “0” and ask to speak with the House Supervisor.
If, after your complaint has been addressed by a Service Representative or Manager, you remain unsatisfied, you may request to have your concerns addressed by the hospital’s Grievance Committee. The Grievance Committee is comprised of representatives of medical staff, administration, nursing, quality management and other hospital departments. Your request to pursue a grievance may be made verbally or in writing to any Service Representative.
Upon receipt of your grievance, the Grievance Committee chairperson will send you a letter advising you of the date of the next committee meeting, as well as specific information as to how your grievance will be reviewed. If you have any questions, you may call the Grievance Committee chairperson.
You will receive a letter dated seven days following the date of the Grievance Committee meeting. This correspondence will advise you of the investigation that has been conducted and the outcome of the Grievance Committee’s review.
You also have the right, at any time during the hospital’s complaint/grievance process, to contact the Department of Health Services, Licensing and Certification Division at 2150 Towne Center Place, Suite 210, Anaheim, CA 92806 or call (714) 456-0630.
You may also contact the Joint Commission’s Office of Quality Monitoring to report any concerns or register complaints about this organization by either calling (800) 994-6610 or e-mailing email@example.com.
