Pre-Registration ProcessScheduled Admission
If you are having a scheduled procedure, there are two ways to pre-register:
- For your convenience, you can pre-register online. Click here to access the online form.
- You may choose to have us call you. If you choose the pre-registration option, you may receive two phone calls prior to your admission date:
- A patient account specialist (from the Admitting Department) may call to gather necessary information, including the correct spelling of your name, date of birth, address, nearest relative, emergency contact, employer name, address, phone number and your health insurance information. The patient account specialist will verify your insurance benefits and will obtain the authorization/pre-certification for your admission. Financial arrangements will be discussed.
- A registered nurse may call to obtain basic health information, discuss instructions from your physician and answer questions you may have about your procedure/hospital stay.
Obstetrics (OB) Admission
Obstetrics patients can also choose to preregister online. Click here to access the online form. Obstetrics patients can also choose to complete the pre-registration packet they receive from their physicians. Once completed, the packet should be returned to:
