Living with Chronic Health Conditions Support Group

Support Groups

The Chronic Conditions Support Group is a safe place for people to get together to share concerns, success stories and coping strategies. This group offers patients social support, and a chance to meet others who can relate to their experiences. Patients can receive help, and help others as they adjust and manager their chronic condition. Chronic health conditions include heart disease, arthritis, multiple sclerosis and many others.
