Requests for copies of your medical record must be in writing, dated and signed by you or your legal guardian. No telephone requests can be honored. Your written request should include the following: full name (including any former name you may have used when obtaining healthcare), birth date, date of treatment and what parts of the medical record you want copied. There is a fee to receive personal copies of a patient’s medical record. The request should be mailed or brought to the Medical Records Department, located on the lower level at St. Joseph Hospital. Medical records will be released after discharge, not during your stay.
