Obtaining Copies of Images

Copies of your images can be obtained at one convenient location:

Hours of Operation: 7 a.m. -11 p.m.
Copies are $10 per CD (cash or check).

For release of your personal medical images or reports: Please bring your ID and the name of your referring physician. You will be asked to sign a Medical Information Release Authorization to release the records to you.

For release of medical images or reports for a friend or family member : Please bring the signed Medical Information Release Authorization, a copy of your friend or family member's ID, your ID and the name of the referring physician.
