1- Call us at (866) 888-9641

• We can help you choose the physician who is best for you and your family.
• We'll give you your health plan phone number and the Provider ID Number to select your physician.
• We'll answer any questions you may have.

2- Call the Member Services Department at your health plan or contact your Human Resources Department at work

• Give them your physician's Provider ID Number
• Confirm with them that you are choosing your physician through St. Joseph Hospital Affiliated Physicians or St. Joseph Heritage Medical Group.
