Day of Discharge

When your physician decides you are ready to leave the hospital, a discharge order will be written. You should make arrangements in advance with a family member or friend to help you when it is time to go home. Here are some important things to remember:

  • Preferred discharge time is 11 a.m., or you can choose a specific discharge time with your healthcare team. It is important to make arrangements for someone to pick you up prior to the arranged time.
  • Personal belongings – Collect all of your belongings and/or medications. If you have anything stored in the hospital safe, call the Admitting Department and they will arrange for your belongings to be brought to you.
  • Discharge instructions – Your physician and nurse will give you instructions regarding your discharge medications and your post-hospital care. If you have questions, please be sure to ask. If there is someone who will be caring for you after discharge, it is a good idea to have this person present.
  • Medication – If medication is prescribed, prescriptions can be refilled at the St. Joseph Hospital Outpatient Pavilion Pharmacy (714-744-8724) or a pharmacy near your home.
  • Escort Service – When you are ready to leave, a hospital representative will escort you to the designated discharge area.

Some patients may need additional treatment or care after they are discharged. If you have any special needs, the case managers will help coordinate your care with your physician and your insurance company (if necessary). These needs may include home health, home medical equipment, acute rehab, skilled nursing care, custodial care, board and care, home care aids and hospice. If you are anticipating after-discharge care, please contact your nurse or case manager.
