| During the admission process, you will be asked for a copy of your insurance card, a form of picture identification (e.g., a driver's license) and a copy of your Advance Directive. If you do not have a copy of your Advance Directive, you will be asked to write the contents of your Advance Directive on an Interim Directive until the original is brought to the hospital. If you have not been asked during the admission process to complete an Interim Directive, as mentioned above, contact the admitting department or your nurse.
You will be asked to sign a Conditions of Admission form. This is your authorization for medical and/or surgical treatment. If you are having surgery, you will also be asked to sign a special consent for your surgery and the administration of anesthetics. If you are a parent for a minor or a legal guardian, you must sign these consent forms.
You will also receive a letter explaining that any physicians' fees will be billed separately from the hospital bill. This includes bills from such specialists as the pathologist, radiologist, anesthesiologist, cardiologist, etc. These specialists are independent physicians and are not employees of the hospital. You will then be escorted to your room. If you are having surgery, you will begin your stay in the Surgical Prep Unit.
Following admission, you will receive a comprehensive initial assessment by your nurse. This includes a physical, psychological, social, spiritual and functional evaluation. This information helps us identify and prioritize your treatment plan. An assessment of your discharge planning needs will also begin within 24 hours of your hospital admission. |
